Exceptions to Confidentiality

• You are committing a crime

Limitations of Liability

Dr. Simpson is not liable for any damages, including but not limited to, direct, indirect, incidental, special, or consequential damages, arising out of or in connection with her services.

Governing Law

This Agreement shall be governed by and construed in accordance with the laws of the State of [State].

Entire Agreement

This Agreement constitutes the entire agreement between Dr. Simpson and you. No other agreements, written or oral, exist between the parties.

Amendments

This Agreement may be amended only by a written agreement signed by both parties.
